Clean the rod thread and the clevis thread with brake clean before fitting, then use a medium strength thread locker such as Loctite 243 as well as the grub screw. The grub screw and nylon insert stop the clevis backing off, they are not what holds the load, so the clevis must be screwed fully home onto the rod shoulder. Line the clevis up square to the base end before you tighten the grub screw. A clevis that is a few degrees out puts a twist in the rod every stroke, and that wears the gland seal and rod bush long before their time.
